New Patient Appointment Checklist
Please bring the following items with you to your first appointment:
- Completed applicable New Patient Forms which can be found below in the New Patient Forms Section
- List of all medications you are currently taking and the dosage
- Insurance Card(s)
- Preferred Pharmacy Name
- Payment Method (cash, check, or major credit card)
- Relevant Lab Reports (P.S.A., semen analysis, urine culture, etc.) if not already on file at Dr. Head's office
- Abdomen and pelvis X-rays, CT scans, and/or MRIs that are not already on file at Dr. Head's office
